Dear Patients and Families,

After much reflection, I am writing to share that I will be retiring after 30 years of practicing integrative and biomedical medicine. It has truly been a privilege and an honor to serve you as your physician and to be a part of your medical journeys.

As part of this transition, please note the following:

• True Health Medical Center will be closed as of Friday, July 31, 2026.

• If you require physical medical records PRIOR to 2014, please complete the attached True Health Medical Center Release of Medical Records form. You can email, mail or fax the completed form to the clinic. Please do so by March 31st, 2026 , so we have time to prepare the paperwork. Note that there will be a charge for copying these records.

• You will continue to have access to your electronic health records in Charm even after the office is permanently closed.

• My hope is to follow up with each of you one more time to help determine the best path forward, assist with medication refills as appropriate, and support you while you transition to a new practitioner. Please contact Eileen via phone or portal message if you wish to schedule an appointment prior to July 31, 2026. All requests will be handled in the order they are received.

Caring for you at True Health Medical Center has been one of the greatest joys of my life. Over the years, I have felt like a part of so many of your families—sharing in your triumphs, walking alongside you through challenges and losses, and celebrating milestones together. Your trust has meant more to me than I can adequately express, and I am deeply grateful for the relationships we have built.

Thank you for allowing me to care for you and for the confidence you placed in me over the years. I will always carry these experiences and connections with me, and I leave this chapter with immense gratitude and heartfelt appreciation.
